Fachsprachprüfung for Schleswig-Holstein: registration and venue

In short

Schleswig-Holstein has no Fachsprachprüfung (FSP, medical German exam) of its own for physicians. The „Fachsprachenprüfung Medizin C1“ is run by the Ärztekammer Mecklenburg-Vorpommern (the neighbouring state's medical chamber) on behalf of Schleswig-Holstein, on its own premises. You cannot register yourself: registration goes through the SHIBB Landesamt in Kiel, the authority responsible for medical licences, and requires residence or a job offer in Schleswig-Holstein. Expect a fee of €700 and, according to the authority, a waiting time of about three and a half months.

Who examines for Schleswig-Holstein?

In Schleswig-Holstein, the Approbation (full licence) and the Berufserlaubnis (temporary licence) are handled by the Schleswig-Holsteinisches Institut für Berufliche Bildung (SHIBB Landesamt), health professions department, Sachgebiet 50 in Kiel. The FSP, however, is not run by the Ärztekammer Schleswig-Holstein — which is responsible for the Kenntnisprüfung (medical knowledge test) in the recognition procedure — but by the Ärztekammer Mecklenburg-Vorpommern. The exam takes place on the premises of the M-V chamber, which is based in Rostock. For candidates from Schleswig-Holstein this means: a different state, a different format, and travel to plan.

A word of caution about older information: the pages of the Ärztekammer M-V and some passages on the state's own pages still name the Landesamt für Arbeitsschutz, Soziales und Gesundheit (LASG) or the former Landesamt für soziale Dienste for Schleswig-Holstein. The current registration form is addressed to the SHIBB Landesamt.

Registration via the SHIBB

  1. Apply for an Approbation or Berufserlaubnis at the SHIBB — in writing or online via the Anerkennungsfinder (recognition finder), not by e-mail.
  2. Register for the FSP with the form „Antrag auf Fachsprachenprüfung“. Attach, unless the SHIBB already has them: the form „Persönliche Angaben zum Anerkennungsverfahren“ (personal details), a certified copy of your B2 certificate, a current, complete and signed CV, and a current registration certificate showing your main residence in Schleswig-Holstein.
  3. Wait for your date. The SHIBB forwards your registration to the Ärztekammer M-V; the chamber contacts you with a date.

Two deadlines make early registration important. If you work with a Berufserlaubnis, according to the SHIBB you must submit a Fachsprachenzertifikat C1 Medizin within six months of starting work — with about three and a half months of waiting time, there is little buffer if the first attempt fails. And for an Approbation after an equivalence assessment, or for admission to the Kenntnisprüfung, the certificate must be available at that point; otherwise the procedure is delayed.

Format: the Ärztekammer M-V exam

You are examined in the Ärztekammer M-V format, which differs markedly from the usual FSP format: 45 minutes of preparation with a briefing and a standardised Aufklärungsbogen (patient consent form), then a 20-minute doctor–patient conversation including informed consent, a 20-minute doctor–doctor conversation and finally 25 minutes for a discharge letter on a computer. A panel of at least two physician examiners assesses your language, not your medical knowledge; you learn the result straight after the exam. Each part, with examples and tips on informed consent and the letter, is explained on Fachsprachprüfung in Mecklenburg-Vorpommern.

Fees: €600 or €700?

The SHIBB states an exam fee of €600 on its information page, but marks the figure „Stand März 2024 – ohne Gewähr für die Richtigkeit“ (as of March 2024 — no guarantee of accuracy). The fee is not charged by the SHIBB but directly by the Ärztekammer M-V — and its fee schedule, valid since 1 January 2025, sets €700 for the Fachsprachenprüfung. So expect €700 per attempt.

On top come travel to Mecklenburg-Vorpommern and the fees of the recognition procedure at the SHIBB (according to the authority, €320 for the Approbation, at least €150 for a Berufserlaubnis). How the chambers' fees compare is shown in FSP fees.

Other language certificates

The SHIBB also recognises C1 medical language certificates from other state medical chambers, „sowie ggf. anderer geeigneter Prüfungsanbieter“ (and, where applicable, other suitable exam providers) — with the explicit request to check before registering with another provider whether its exam will be recognised. It publishes no list of recognised providers; the current information page does not name the telc exam explicitly. How telc and FaMed exams differ from the chamber exam is explained in Recognition of telc and FaMed.

Bear in mind, too, that several chambers only examine applicants from their own state — Saxony-Anhalt, for example, requires a future workplace in the state, Rhineland-Palatinate an application at its own state authority. Going through another chamber is therefore no reliable shortcut.

Retakes

The Ärztekammer M-V rules apply: retake of the whole exam only, at the earliest after three months, with no limit on attempts. Applicants from Schleswig-Holstein request a retake not from the chamber but through the state authority. The chamber still refers to the LASG for this, and among the SHIBB's forms there is a retake form only for the Kenntnisprüfung, not for the FSP. So clarify the route directly with the SHIBB (hotline 0431 988-9126 or [email protected]).
