FSP documentation: filling in the Anamnesebogen (with sample)

In short

In part 2 of the Fachsprachprüfung you summarise the history-taking conversation in writing in about 20 minutes — in most medical chambers on a pre-printed Anamnese-, Berichts- or Aufnahmebogen (history, report or admission form): header fields at the top (patient, allergies, substance use, social and family history), below them, in full sentences, the history, suspected diagnosis, differential diagnoses, further diagnostics and a therapy proposal. There is no findings section, because nobody is examined in the FSP. What is graded is language: faithful rendering of what was said, grammar, spelling, medical terminology and completeness — not whether your diagnosis is right. Only Brandenburg, Mecklenburg-Vorpommern (also for Schleswig-Holstein), Saxony-Anhalt and Bremen explicitly ask for free text instead of a form.

Which chamber asks for which document

There is no nationwide standard form: each chamber decides what the part-2 document looks like. The table summarises what the chambers publish; “not stated” means the chamber says nothing about it — check your invitation or ask.

Chamber Document Writing Aids
Baden-Württemberg Arztbericht (report) on an Anamnesebogen not stated medical dictionary
Bavaria short report on a structured template, second part in full sentences laptop (German keyboard) or by hand no personal aids
Berlin short report on a structured template, at least the current history in full sentences computer or by hand medical dictionary
Hamburg prescribed form by hand — legibility counts none
Hessen Anamnesebogen, up to 20 minutes on the form no aids
Lower Saxony Berichts-/Aufnahmebogen, second part in full sentences by hand, “legibly” medical dictionary
Nordrhein the chamber's Anamnesebogen not stated no other aids
Westfalen-Lippe Berichtsbogen, full sentences “where indicated” by hand or PC medical dictionary
Rhineland-Palatinate four-page Anamnesebogen (20 min) plus an examination request form (5 min) by hand — illegible text costs points not stated
Saxony structured form, full sentences not stated medical dictionary
Thuringia Arztbericht in full sentences not stated medical dictionary
Brandenburg Arztbrief (doctor's letter), 30 minutes the chamber's laptop not stated
Mecklenburg-Vorpommern, also for Schleswig-Holstein Entlassungsbrief (discharge letter), 25 minutes, as the last part computer not stated
Saxony-Anhalt Epikrise (clinical summary) after working through a patient file, 20 minutes computer without spell-checker medical dictionary
Bremen short report as free text not stated not stated

Thuringia does not say whether its “Arztbericht” is written on a template. Saarland holds no exam of its own. How to build a free-form letter for the last four chambers is covered in The Arztbrief in the FSP.

The published sample forms

Three chambers put their form online. It is worth printing it once before the exam and filling it in with a practice case — then you know the order and the space you have.

Nordrhein (Muster-Anamnesebogen): a header with “Patient/Patientin (Vorname, Name, Geb.-Datum)” (first name, surname, date of birth) and the date; on page 1 a large box “Aufnahmegrund/Diagnose (stichwortartig)” — reason for admission/diagnosis in keywords, so keywords are explicitly allowed here. Page 2 has allergies/intolerances, substance use, social history and family history, then a block “Verdachtsdiagnose/Differentialdiagnose/weitere Diagnostik/Therapievorschläge” with the note “Bitte benutzen Sie Fachwörter, wo möglich” (please use medical terms where possible); the block continues on page 3.

Lower Saxony (Berichtsbogen): page 1 asks for patient, date, age (years), height (cm), weight (kg), allergies/intolerances, substance use, social history and family history. Page 2 is an empty box with the instruction: “Schreiben Sie eine Zusammenfassung des Anamnesegesprächs in Form eines Berichts mit Anamnese/Verdachtsdiagnose/Vorschlägen zur weiteren Diagnostik und Therapievorschlägen. Angaben der ersten Seite müssen hier nicht wiederholt werden. Bitte formulieren Sie in ganzen Sätzen!” — write a report with history, suspected diagnosis and proposals for diagnostics and therapy; do not repeat page 1; write in full sentences.

Rhineland-Palatinate (four-page Anamnesebogen): page 1 holds the patient's name, date of birth, age, weight, height, place of residence, GP, occupation, marital status, children and “Aktuelle Anamnese: (Bitte schreiben Sie ganze Sätze)” — current history in full sentences. Page 2 asks for previous illnesses and operations, family history and gynaecological history, page 3 for the review of systems (temperature, sleep, appetite, bowel movements, micturition), mental state, medication, substance use and allergies, page 4 for the suspected diagnosis, differential diagnoses and the investigations discussed with the patient. You receive the same form for the conversation itself.

The other chambers only describe their template in words. The pattern is similar everywhere: header fields for data and secondary histories, then a report running from the reason for admission to the therapy proposal.

No findings: what does not belong on the form

The FSP tests a conversation, not an examination. That is why no published form has a findings field, and Bremen says so explicitly: “Ergänzen Sie keinesfalls ‘erdachte’ Textblöcke wie beispielsweise theoretisch mögliche Beschwerden, einen körperlichen Untersuchungsbefund oder Befundberichte” — never add invented text such as hypothetical complaints, a physical examination or test reports. What you want to examine goes under “weitere Diagnostik” — as a plan, not a result: “Geplant ist eine Sonographie des Abdomens” (an abdominal ultrasound is planned), not “Sonographisch Gallensteine” (gallstones on ultrasound).

Exceptions exist only where the chamber itself hands out documents: in Saxony-Anhalt you work through a patient file with imaging and lab results before writing the Epikrise. In Nordrhein the examiners may present further documents in part 3, and Berlin has you read out lab values in part 3 — neither concerns the form.

Full sentences, keywords, terminology: the chambers' rules

Most chambers want the report in full sentences: Bavaria and Lower Saxony “in the second part of the template”, Berlin “at least the current history”, Westfalen-Lippe “where indicated”, Saxony and Thuringia throughout, Bremen additionally with “gegliederte Satzstrukturen” (structured, complex sentences). Header fields such as allergies or family history can be filled in briefly almost everywhere.

Rhineland-Palatinate has the most detailed rules — and different ones. The Rheinhessen district chamber's notes require the current history on page 1 in full sentences, but explicitly no full sentences on pages 2 and 3 (“Nichtbeachten führt zu Punktabzug” — ignoring this costs points). Also: “Übersetzen Sie keine Patientenangaben in die Fachsprache (Eine Anamnese ist kein Arztbrief). Verwenden Sie nur auf Seite 4 die Fachbegriffe.” — do not translate the patient's statements into medical terminology (a history is not a doctor's letter); use medical terms only on page 4. On the examination request form, the questions to the colleague are in medical language, the clinical details “so, wie vom Patienten berichtet” (as reported by the patient). And: “Alle Fakten aus dem Gespräch müssen als Nachweis des richtigen Verstehens aufgeschrieben werden” — every fact from the conversation must be written down as proof that you understood it.

Elsewhere medical terms are welcome, but controlled: Nordrhein asks for them in the diagnosis block, Berlin grades “korrekt angewandte fremdsprachliche Fachwörter in angemessenem Umfang” (correctly used medical terms in appropriate measure), Saxony warns “Geben Sie exakt das Gesagte wieder, interpretieren Sie nichts” (reproduce exactly what was said, interpret nothing). In practice: translate only what is unambiguous (“Übelkeit” → Nausea, “Kaiserschnitt” → Sectio caesarea). If the medical term turns a statement into a diagnosis (“Herzstolpern”, palpitations → Vorhofflimmern, atrial fibrillation), keep the patient's wording, in quotation marks if need be. The patient's statements are written in reported speech — the rules are in Konjunktiv I in FSP documentation.

How Berlin grades the documentation

Berlin is the only chamber that publishes its scoring sheet for part 2. Eight criteria, each worth 0 to 2.5 points; “Das erstellte Dokument …” (the document …)

  1. “… ist der/dem Simulationspatient:in sicher zuzuordnen.” — can be attributed to the simulated patient without doubt (only 2.5, 1.5 or 0 points)
  2. “… ist strukturiert und vollständig abgefasst.” — is structured and complete
  3. “… ist grammatisch korrekt.” — is grammatically correct
  4. “… ist orthografisch korrekt.” — is correctly spelt
  5. “… zeigt die nötige Detailtiefe.” — shows the necessary level of detail
  6. “… enthält korrekt angewandte fremdsprachliche Fachwörter in angemessenem Umfang.” — uses medical terms correctly and in appropriate measure
  7. “… gibt die wesentlichen Anamnesebefunde semantisch korrekt wieder.” — renders the essential history findings with correct meaning
  8. “… trifft klare Aussagen über die wesentlichen Behandlungsschritte.” — makes clear statements about the essential treatment steps

The maximum is 20 points; the part is passed from 12. Because Berlin requires 60% in every part, weak documentation alone can fail the exam. Roughly half of the criteria concern language and form, the other half content — and criterion 8 shows that a report without a therapy proposal loses points. More on grading all three parts is in How the FSP is graded.

Sample: a completed report form

Example — fictional case. It is based on the practice case “Birgit Lüdemann, 54” from fsppruefung.de (pain in the right upper abdomen since the previous evening), entered in the layout of the Lower Saxony Berichtsbogen. The medical content is language material, not a treatment recommendation. The form stays in German, as in the exam.

In English, in brief: a 54-year-old woman with constant right upper abdominal pain since a fatty meal the previous evening, radiating to the right shoulder, with nausea, vomiting twice, 38.6 °C and one episode of rigors, three similar short episodes last year; suspected acute cholecystitis with gallstones, differentials, planned tests and a treatment proposal.

What this sample gets right

The same case on other forms

On the Nordrhein form the reason for admission may be written in keywords, for example: “Seit Vorabend anhaltende Schmerzen re. Oberbauch nach fettreichem Essen, Ausstrahlung re. Schulter; Übelkeit, 2× Erbrechen; 38,6 °C, 1× Schüttelfrost; 3 ähnliche Episoden im Vorjahr.” Suspected diagnosis, differentials, diagnostics and therapy follow in their own boxes, with medical terms. In Rhineland-Palatinate the current history on page 1 would be in full sentences and without medical terminology; page 2 would read briefly “Zucker seit 6 Jahren (Tabletten), Bluthochdruck, erhöhte Blutfette; Kaiserschnitt 1995” (lay words for diabetes, high blood pressure, raised blood lipids, caesarean section), and only page 4 would say “akute Cholezystitis bei Cholezystolithiasis”.

On fsppruefung.de you write part 2 on a chamber-style documentation form — using the notes you took during the conversation.
